A NEW CHAPTER BEGINNING OCTOBER 19, 2026

A more personal
approach to
your care.

After three decades of caring for this community, I am transitioning to a concierge medicine model — a smaller practice built around unhurried visits, direct communication, and the perpetual doctor-patient relationships that have always been central to my care.

A NOTE TO MY PATIENTS

"Beginning October 19th, 2026, my practice will be changing to a concierge medicine model, allowing me to spend more time with each patient and strengthen the doctor-patient relationship while preserving the personal approach that has always been important to me."

— William V. Jacobson, MD

Membership Benefits

Fewer patients. Stronger relationships.

The concierge practice will be smaller and I will intentionally have a limited number of patients. A membership fee supports the enhanced availability, personalized care, and additional services this model provides.

Unhurried Visits

Longer appointments designed for real conversation, careful listening, and thorough evaluation — never rushed.

Direct Physician Line

Direct communication even outside of business hours, so you're speaking with your doctor when it matters.

Same or Next-Day Scheduling

When you need to be seen, you will be seen — promptly, and by the physician who already knows you.

Personalized Wellness Planning

A comprehensive annual visit and an ongoing plan built around your goals, history, and family circumstances.

Please know that my office will remain in the same location, and I will continue to be an in-network provider with Medicare and most commercial insurance plans. The membership fee is separate from your insurance and does not replace insurance coverage or cover office visits, deductibles, or copayments.

I've always believed that every patient deserves to be treated with compassion, respect, and the kind of attention I would want for my own family.

My practice has never been about seeing as many patients as possible. It's about providing thoughtful, personalized medical care and building relationships that last for years.

It has been an honor to care for my patients over the past three decades, and I look forward to continuing to do so with the same commitment, compassion, and personal attention that have always guided my practice.
