Concierge Care
Medicine with the time it deserves.

Concierge medicine is a simple idea: Dr. Jacobson cares for fewer patients so he can care for each one more completely. In practice, that means longer visits, a direct line to your doctor, appointments when you need them, and someone who is your continued health partner.
What membership means for you

Longer, unrushed visits
Appointments are scheduled with room to breathe. There is time to review your history, talk through concerns fully, and examine carefully.

Direct communication
You reach Dr. Jacobson directly, including after hours. Questions get answered by the physician who knows you and your history.

Same or next-day visits
When something comes up, you are seen promptly, typically same or next business day, by your own physician instead of an urgent care stranger.

Care coordination
Dr. Jacobson coordinates with specialists, shares your history directly, and follows your care through. He is affiliated with White Plains Hospital.
The membership fee is separate from your insurance. Dr. Jacobson remains an in-network provider with Medicare and most commercial plans, and your coverage continues to work exactly as it does in a traditional practice.
Common Questions
Questions patients often ask
What does concierge care actually change for me?
The practice is intentionally small, so appointments are longer and easier to schedule. You see Dr. Jacobson himself, he already knows your history, and there is time in every visit to talk through what is on your mind rather than a single issue at a time.
Can I still use my insurance?
Yes. Dr. Jacobson remains an in-network provider with Medicare and most commercial insurance plans, and your insurance continues to be billed as usual for covered medical services. The membership fee is separate and supports the enhanced availability and personalized services of the concierge practice.
What happens if I need a specialist or a hospital stay?
Dr. Jacobson stays involved. He helps identify the right specialist, shares your history directly with them, and follows your care through so that nothing gets lost between offices. He remains affiliated with White Plains Hospital.
What if something comes up at night or on a weekend?
You are able to reach Dr. Jacobson directly, including outside of normal business hours. Many concerns can be settled with one conversation with the physician who already knows you, saving a trip to urgent care or the Emergency Room.
Is the office moving?
No. The practice remains at 311 North Street, Suite 202 in White Plains.
